- Total cost and what it included
- What went well and what was hard, in your own words
What we never collect
- Full names, dates of birth, record numbers or photos
- Medical files in this form; verification comes later, by a separate channel
Consent, in plain words
- You are the patient or an authorized family member
- You agree we may edit and publish a de-identified version
- You understand it is an individual experience, not medical advice
- You disclose any compensation you received
